The Small House Model has proven to be the new standard of excellence in skilled nursing and rehabilitative care!
• A home-like environment with personal choices for wake-up, bedtime, meal selection, activities and rehabilitative care
• Now 12 elders (residents) per house, dramatically improving personalized care, socialization, and a proven track record positively impacting infection control (including COVID-19)
• Dedicated caregivers for each house, establishing ongoing relationships with each elder
